Description:

We are seeking a Therapy Operational AHP lead to work within our MDT at the community hub in Gorton. This dynamic new role will improve patient pathway on discharge from hospital and ensure best patient experience of rehabilitation.
You will provide excellent AHP operational leadership to our specialist community rehab team and Manchester Response teams including Discharge to assess.
The rehab and Manchester Community Response services work collaboratively, enabling patients to receive safe treatment at home. We aspire to enhance the “Home first” agenda that supports meeting care needs and providing therapy at home.
Using your experience and knowledge, you will support team members to ensure a high-quality patient service, providing education and support as needed.
You will lead on planning, delivery and evaluation of CPD, clinical audit, research and training.
You will coordinate student placements for Physiotherapy and Occupational therapy for all MCR services.
As a team member you will receive regular supervision, training, and support to pursue post graduate education.
You will be involved in projects and trust initiatives giving the opportunity to form professional, multi-disciplinary relationships.

To be the operational team lead across 3 highly specialised Community Rehabilitation and MCR services.
The post holder will manage and provide leadership for the specialist Community Rehabilitation team, the Discharge to Assess Team and the Intermediate Home Care Pathway Team.
As an experienced clinician with an advanced level of knowledge and skills, support the teams in assessment, diagnosis and treatment of complex and acute patients.
To plan, deliver and evaluate Continual Professional Development, clinical audit, research activities and in-service training within specialist area.
To lead clinical service development within specialist field, working closely with colleagues across central adult and specialist community services.
