Description:

As a member of the dementia service the post holder will contribute to the delivery of patients’ individual care under the direction and supervision of the Dementia Clinical Specialist (DCS)
Gather quality collateral history/ personal information from patients, families, carers and other relevant services (eg social worker, care agency)
Deliver, monitor and embed DCS care plans for patients with dementia using clinical leadership skills to role model best practice to ward staff
Work closely with dementia service colleagues and contribute to service goals, taking responsibility for managing and collecting and inputting data metrics
Use clinical judgment to feedback to DCS patient progress and assist in setting patient goals that reflect their changing needs
Ongoing carer communication/ liaison, including referrals to community resources where indicated (eg carer support organisations)
Facilitate the provision of meaningful activities for patients, working closely with band 2s and volunteers
Facilitate supported/ risk-positive discharges
