We are looking for an exceptional pharmacist to join a new service being established within the Manchester Local Care Organisation called Manchester Case Management (MCM). This is an exciting time to be part of our developing and progressive working environment.
MCM is a multi-skilled service which proactively supports people identified as “at high risk of hospital admission”. The service brings together the skills and expertise of GPs, nurses, mental health nurses and practitioners, social workers, pharmacists, pharmacy technicians and wellbeing advisers to provide intensive and flexible support to people who are struggling to stay healthy, safe and well at home. The service shares the care of patients across the disciplines, providing support through clinic appointments, home visits and telephone calls. The service will be embedded in the three localities; North, Central and South Manchester, being delivered through the neighbourhood model.
The MCM pharmacists will be supported by the MLCO Community Medicines Optimisation Service, a friendly team located in the community setting. We are an award winning team at the forefront of the local integration agenda, developing varied and innovative new projects and ensuring our patients are supported to get the very best from their medicines. Although community based, the team works closely with the Manchester Foundation Trust (MFT) Pharmacy Department. As such we benefit from working for a large, forward-thinking department, including access to training programmes and excellent career-development opportunities.
You must be motivated, enthusiastic and have experience of working in a multi-disciplinary team. You must also have a significant amount of hospital or primary care experience, excellent people skills, attention to detail and good time management. You will need to demonstrate good written and verbal communication skills, have good organisational skills and an ability to work independently and as part of a multidisciplinary team. You will also need to be registered with the GPhC, able to drive and have access to a car.
In return we will provide a comprehensive induction programme and you will be appraised on a yearly basis with a view to developing knowledge and skills where appropriate.
This post is 18.75 hours per week but applications from pharmacists wanting different hours will also be considered.
