Description Work Location: KID’S HEALTH CONNECTION Detroit, MI Employment Type: Part-time with Benefits Classification: Non-Exempt Duration: 3 Month Assignment Salary: $18.50 per hour Position Summary The Community Health Worker (CHW) at SEMHA will play a key role at the Kids’ Health Connection program, serving as a front-line resource for families with children ages 0–4, that focuses on addressing early childhood health needs and reducing healthcare disparities across the metro-Detroit area. This is a 3-month grant funded part-time assignment with benefits, that may be extended. The CHW serves as a trusted community liaison, working closely with families, healthcare providers, insurance payors, and community organizations to improve access to care, promote healthy development, and support overall family well-being. Key Responsibilities Partner with families, healthcare providers, schools, and community agencies to ensure coordinated, high-quality care and avoid duplication of services Educate families on appropriate emergency room usage, well-child visits, immunizations, and preventive healthcare practices Assist families in accessing essential resources, including transportation, housing, food assistance, and behavioral health services Support families in understanding care plans and medical instructions Encourage and empower families to actively participate in their health and well-being Coordinate appointments and facilitate access to healthcare services Engage community members to gather feedback and improve program effectiveness Maintain accurate, confidential participant records Attend required training and maintain program compliance Demonstrate professionalism in all interactions Qualifications Community Health Worker Certification issued by the State of Michigan (required) Valid driver’s license, current auto insurance, and reliable transportation (required) Ability to attend all required program trainings Education & Experience High school diploma or equivalent required; Associate or higher preferred Minimum of two (2) years of relevant work experience Experience in a healthcare or community-based setting strongly preferred Proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint) and general computer skills Experience with digital engagement tools (e.g., Canva, Instagram, Facebook) preferred Strong organizational skills with attention to detail and documentation accuracy Self-motivated with the ability to work independently and manage multiple priorities Application Deadline: March 24, 2026.
