Description:

We have an exciting opportunity available in Camden Integrated Community Healthcare Team. This would be suitable for a new graduate with an interest in working in the community or physiotherapist with previous experience, who wants to consolidate their skills in the area of community rehabilitation. Previous experience of working in a community setting is desirable but not essential. This is a permanent post is based at Gospel oak health centre which is part of North neighbourhood.
You will be working alongside senior Physiotherapy colleagues, Occupational Therapists, community Nurses and a range of other AHPs providing home & community based rehabilitation programmes in a diverse and exciting central London borough.
As of 1st July, we have entered the new Integrated Neighbourhood Team working model where there is integration with AHPs, DN’s, social services and other services to provide joined up care to meet needs of local population. However there is an expectation from the post holders to work across neighbourhood boundaries within Camden to support wider staffing.
The posts will give the successful applicant opportunity to experience a wealth of learning and development and to work alongside a responsive dynamic team who put patient centred care at the heart of what they do. We work in an inter-disciplinary way to provide client-centred, goal focused therapy to patients in the community.
You will have an interest in working as part of a team to provide high quality, patient centred interventions. The caseload is comprised predominantly of adults who may present with multiple complex health needs. As such, you will have the opportunity to develop a wide range of clinical skills and interests in areas such as falls, orthopaedics, general medical and surgical conditions. An understanding of the management of multi-pathology patients and the impact that psychological/cognitive factors may have is desirable.
The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.
The service embraces innovative and evidence based ways of delivering services and strives to provide an efficient and high quality service for all. We have excellent relationships with our local partners in primary and secondary care.
You will have a robust CPD plan in place alongside the opportunity to work closely with District nurses, complex care nurses, falls educational and group exercise programmes.
You will be provided with the opportunity to develop your clinical skills along a broad spectrum of pathways and also leadership skills through the support of your senior colleagues. As a Band 5 Physiotherapist, you will receive mentorship and join a preceptorship programme to help your career flourish.
You are responsible for a designated caseload of patients, ensuring their rehabilitation programmes are patient centred to enable them to reach their goals.
As a band 5 you will have the opportunity to represent the team at GP meetings, neighbourhood events and develop relationships with our local community partners in health and care.
You are expected to participate in training and development opportunities, weekly supervision and appraisal
