Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996.  Community Healthcare of Texas has cared for patients with serious and terminal illnesses throughout North Central Texas.  Providing compassionate care for those living with an illness while supporting those caring for a loved one is the mission of Community Healthcare of Texas. POSITION SUMMARY Meet with patients and families to explain the scope of hospice services, coordinate admission of patients and ongoing plan of care.  ESSENTIAL FUNCTIONS * Assess patient's physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.  * Provide emotional support to patients and families while instructing them in activities of daily living.  * Complete daily paperwork and charting, thoroughly and accurately according to Policy.  * Evaluate the patient for hospice appropriateness and complete admission visit and documentation as needed; communicate with the Case Manager on significant findings.  POSITION QUALIFICATIONS * Registered Nurse in the State of Texas  * Hold a valid driver's license and have a dependable means of transportation.  * 2 years of nursing experience required   * 2+ years of experience in hospice strongly preferred  * Proficient skill using a computer is required; Microsoft Office Suite experience preferred.  Reasonable Accommodations Statement  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
