PT POSITION SUMMARY:

Responsible for evaluation, plan of care, treatment, re-evaluation, discharge, documentation, and modification of all client programs. Administers physical therapy in compliance with Federal, State, local, and Bethany Village Home Health Care standards, policies, and guidelines.

QUALIFICATIONS:

  • Current Ohio Licensed Physical Therapist
  • CPR certification.
  • Valid drivers’ license and a safe driving record.

OT POSITION DESCRIPTION:

Responsible for evaluation, plan of care, treatment, re-evaluation, discharge, and appropriate and effective communications to clients.

QUALIFICATIONS:

  • Master’s degree or doctorate from an accredited Occupational Therapy program.
  • Current Ohio License in Occupational Therapy.
  • CPR certification.
  • Valid drivers’ license and a safe driving record.

Responsibilities

JOB DUTIES:

  • Administers physical therapy in compliance with Federal, State, local and Bethany Village Home Health Care standards, policies, and guidelines ensuring quality care and service to clients.
  • Interacts with health professionals, clients, families, and support groups to ensure maximum rehabilitation, while serving as a client advocate at all times.
  • Evaluates clients by administering muscle, neurological, gait, and functional tests.
  • Establishes treatment goals based on the evaluation of the client in conjunction with the client’s needs.
  • Develops treatment programs and provides ongoing assessments and modifications as changes in the client’s condition occur.
  • Prepares, transmits, and maintains timely and accurate records to include the initial evaluation, progress notes, discharge summary, and attendance records in accordance with agency policy and that support charges for services provided.
  • Promptly reports incidents, accidents, changes of condition, abuse, concerns to the assigned staff nurse or the Director of Bethany Village Home Health Care .
  • Complies with OSHA standards and ensures the safety of the client in their care at all times.

DUTIES:

  • Administers occupational therapy in compliance with Federal, State, local and Health at Home standards, policies, procedures, and guidelines.
  • Implements strategies and interacts with staff members and other healthcare professionals to ensure efficient and quality care to clients.
  • Interacts with clients, families, support groups, staff member, and other health professionals to ensure maximum rehabilitation to the resident while serving as resident advocate.
  • Completes Occupational Therapy evaluations and treatments, plan of care, assessments, and discharge orders that support charges for provision of services in a timely manner.
  • Must comply with OSHA standards and ensure the safety of the clients under their care at all times.
  • Analyzes data and provides recommendations for clients’ care and home assessments.
