Description:

This is the perfect opportunity as a dual role to be part of a diverse team providing a new out of hours home visiting service. As a UTC clinician you will be considered a key person in day-to-day clinical leadership and expertise within the department. You will be working as part of a team within your sphere of competency in adiverse, multi-skilled teamthat includes GPs, Advanced Practitioners, Physician Associates, Paediatric ANPs, Emergency Practitioners and Live Well Coaches. As an OOH HV ECP you will be considered a key person in clinical leadership and expertise within the service.
As this is a dual role, that will primarily be seeing patients within the UTC, however when a visit comes through then you will need to go on the home visit. You will be working autonomously within your sphere of competency as a clinicianin adiverse, multi-skilled team. Your role will require you to respond to 111 referrals in which you will visit patients at their homes to provide care. You will have clear methods to contact and refer to various primary and secondary care services with locally agreed and developed management pathways.
To support you in this role, a GP will be based at one of our clinical sites to provide virtual support in assessment, diagnosis and treatment decision making. You will need to an independent prescriber, as some patient may require medications to be given from the stock in the OOH visiting car, as well possible palliative care patients. The Out of hours visiting service usually commences from 7pm to 8am Monday to Thursday, and 24 hours over the weekend. The Urgent treatment centre is open 24 hours per day.

Principal duties and responsibilities - Take responsibility for the patients assigned through the NHS 111 referral process. - Perform assessment of patients needs, plan, implement and evaluate clinical treatment and care using advanced clinical knowledge and skills. - Be able to discuss with patients and carers the range of possible treatment/ management options and their implications before treatment decisions are made. - Instigate and implement clinical management and treatment changes when appropriate.

  • Contribute to specific care guidelines and standards supporting the care of patients across the health economy. - To implement Health and Safety and Risk Management appropriate to area of work - To deliver aspects of the Clinical Governance Agenda, following the direction of the Clinical Leadership. - Contribute to and manage various elements of auditing in the OOH HV service. Communication and Relationship Skills - Demonstrate excellent communication skills when working with the internal team as well as external, particularly our practices and the hospital trust.
  • Be able to identify and take into consideration barriers to understanding and employ appropriate methods to ensure effective communication particularly where barriers are clinical in nature. - Provide emotional support to patients and carers - Be aware of the procedures to follow if concerned regarding the protection of children. - Ensure compliance with policies, procedures and clinical guidelines for self and others. - Hold good relationship building skills to develop strong relationships with key individuals within practices and the local healthcare system - Have the ability to successfully make appropriate in hospital specialty referrals.
  • Recent post registration experience in clinical assessment - Evidence of professional updating - Up-to-date Basic Life Support Certification in Adult and Paediatric - Minimum of 2 years post qualifying experience - Understanding of patient flow. - Up to date Safeguarding Adults and children - A Proven ability to assess patients - Ensure that patients with time-critical and time-sensitive illness or injury are prioritised accordingly - Ability to demonstrate effective management and leadership skills - Analytical and Judgmental Skills - Perform specialist assessment of patients needs, diagnose where appropriate, plan, implement and evaluate clinical treatment and care using advanced clinical knowledge and skills. - Be able to discuss with patients and carers the range of possible treatment/ management options and their implications before treatment decisions are made. - Be responsible for monitoring health, safety and security of self, the team and others and promote best practice.
  • Identify clinical issues and incidents within the service that reduce the quality of care within the specialist service bringing any issues to the attention of the management team. - Take part in activities to counter these to ensure the delivery of safe and effective care. - Ensure that high standards of care are given and maintained and act when standards are not being met. Planning and Organisational Skills - Plan, organise and prioritise own daily workload.
  • Recognise, prevent, and diffuse potential areas of conflict and where required take a key role in the management of the outcome. - To exercise good personal time management, punctuality, and consistent reliable attendance. Physical Skills - Demonstrate competence in the use of all clinical equipment used in the overall care of patients within the scope of service. - Demonstrate competence in physical examination assessment skills applicable to scope of service.
  • Have competent computer / keyboard skills including ability to utilise software programmes required to provide the service. - Ability to travel to all sites of service delivery in a timely manner. Patient/Client Care - Perform assessment of patients needs, diagnose where appropriate, plan, implement and evaluate clinical treatment and care using advanced clinical knowledge and skills. - Establish effective communication with patients and carers / relatives.
  • Deliver specialised, evidence-based advice suitable for the individuals medical, social, psychological, religious, and cultural requirements. - Ensure that appropriate action is taken to protect vulnerable patients and that they can access/receive the appropriate care. - Ensure compliance with policies, procedures and clinical guidelines for self and others. - Promote peoples equality, diversity, and rights.
  • Identify clinical issues and incidents within the service that reduce the quality of care bringing any issues to the attention of the Clinical Lead. - Take part in activities to counter these to ensure the delivery of safe and effective care. - The post holder is responsible for always ensuring the safety of patients and promoting a safety culture through the effective management of risk. Responsibilities for Policy and Service Development - Contribute and influence policy making and clinical guidelines internally and externally, necessary to support the service.
