Job Summary: Provides care to medically complex patients in collaboration with the physician in a specialized acute care setting. Works within scope and experience and works autonomously and has authority, responsibility, and accountability for assessment, planning, implementation and evaluation of wound, ostomy, and continence care program throughout the inpatient care areas. Serves as a resource for wound care management, including identification, assessment, treatment, and prevention. Within scope and experience, Pprovides specialized consultation on wound, ostomy, and continence issues for patients, families, nursing and medical staff. Serves as a consultant in the management of patients with draining wounds, fistula and tubes. Serves as a consultant for the treatment of patients with altered skin integrity due to pressure necrosis, drainage or incontinence. Works with physicians and nursing staff to provide updated information regarding wound care. Conducts rounds on all patients with wound/pressure ulcers and pressure injuries on an as needed basis. Job Requirements: Education and Work Experience: * Bachelor's Degree in Nursing (BSN): Preferred * Relevant clinical nursing experience: Preferred Licenses/Certifications: * Registered Nurse (RN) licensure in the state of practice: Required * Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Required * Wound care certification: Preferred with plan to achieve in determined time period Facility Specific License/Certifications: * AHA-BLS or ARC-BLS: Required Essential Functions: * Collects relevant data pertinent to the patient’s health or situation. Analyzes the assessment data in determining diagnosis and care issues. Develops a plan that prescribes interventions to attain outcomes. Implements the plan, coordinates care delivery, and employs strategies to promote health and a safe environment. Evaluates progress toward attaining outcomes. Identifies outcomes for the patient or the patient’s situation. * Collaborates with the team of patient, family, and healthcare providers in providing patient care in a safe, healing, humane, and caring environment. Provides learning opportunities for patients/family members and team members. Directly provides health information to patients, families, and treatment team. * Maintains appropriate supply of equipment and materials. Maintains complete, accurate, concise, legible documentation of patient status, interventions and responses. * Performs other job-related duties as assigned.
