Description:
Assist persons with developmental disabilities to take part in activities of their choice, to give a reprieve to the parent/guardian/caretake from caretaking duties while the individual takes part in the program that fosters the opportunity of learning through athletics, arts, crafts, and community involvement.
Responsibilities:
- Provide door-to-door transportation to scheduled activities (if applicable)
- Promote self-advocacy and choice
- Assist individual in making informed choices
- Promote self-determination and community inclusion
- Keep the individual’s aspirations the focal point of their actions
- Take all reasonable steps to ensure the health and safety of the persons receiving services
- Participate in the Life Plan meeting at least every six months (if applicable)
- As outlined in the Staff Action Plan (SAP), assist individuals in acquiring, retaining and improving self-help skills socialization, and adaptive skills necessary in the home and community-based settings
-Complete all services documentation under ADM# 2005 - 02
- Take all reasonable steps to ensure the health and safety of individuals
QUALIFICATIONS
- At least 18 years of age
- Computer knowledge, including but not limited to, Windows and Microsoft Office
- Knowledge of general office procedures and organizational skills
- Ability to work as part of a team with colleagues and other provider services
- Cultural sensitivity and the ability to relate/work with diverse groups, community agencies, schools and the public
- Strong organizational skills with a focus on detail
- Ability to follow directions and strong communication skills (oral and written)
- Ability to pass the NYS Justice Center fingerprinting requirements and background checks
