Description:

To prioritise all admissions both planned and emergency to make best use of the beds available, maintaining effective and safe flow across the Trust To inform medical teams and senior management when beds are unavailable for emergency admissions and offer advice on action to take. Follow bed escalation as per Admissions Policy. To keep senior management informed if problems arise with bed availability by following the Trust Bed Escalation process and act on any advice given. Liaise with Administrative and medical staff when requested planned admissions dates are not available, offering advice as necessary.
Work closely with the infection control team, informing them of any high risk patients admitted to the hospital, and to facilitate the movement of any newly diagnosed high risk inpatients within the hospital to the correct location. To work at all times within the hospitals Operational Polices regarding bed management To work with the ward teams to promote proactive discharge planning for all inpatients. Arrange for patient transfer to other wards as outliers when there is an increased demand for beds which exceeds capacity in any speciality at any given time. Manage the requests for inter hospital transfers and repatriations, working closely with medical teams to identify clinical priority.
Update bed management documentation to provide an accurate record of bed management activity and issues. Take responsibility for the supervision of patients waiting for admission in the department, respond appropriately to changes in condition. To ensure all patient awaiting admission are kept informed of any delays in bed availability. Work collaboratively within the clinical areas to ensure admission and discharge procedures are adhered to.
Contribute to the achievement of Trust targets, whilst identifying service development priorities. Participate in the implementation of Clinical Governance systems
