Description:

This is an exciting opportunity for to be part of the Gedling Local Mental Health Team as a community Support worker. If you are seeking to develop their skills and expertise in working with adults living in the local community experiencing severe mental health problems. You will be an integral member of a dynamic multidisciplinary team offering creative and consistently high quality evidence-based interventions
You will have opportunity to access training, and to develop skills in working within a specific pathway, for example with patients experiencing personality disorders, or rehabilitation of patients experiencing ongoing psychosis.
The service is for working age adults with mental health needs and is delivered in their own homes. You will contribute to assessments and interventions, undertaking nursing, therapy and social activities as delegated by the relevant profession. You will frequently work without direct supervision in the community and be required to work in a flexible and holistic way in order to respond to this client group’s needs. You will promote the recovery of patients and through working with partner agencies and primary care to ensure timely and safe discharge from secondary care.
We are seeking enthusiastic team players, to work within a dynamic multi-disciplinary health team, with excellent communication and interpersonal skills, and the ability and experience to manage complex cases in partnership with other agencies.

You must have evidence of a good basic education and basic IT skills. You will require good verbal and written communication skills and previous experience of working in a caring environment and working as part of a team. Some knowledge of the mental and physical problems associated with older people and the risks associated with community working are required. You will need to have good prioritization skills and be able to recognize your own limitations. Recent significant experience of caring for older adults with mental health needs is very desirable for this post.
A driving licence is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
