Description:

We are looking for a highly motivated, driven individual with extensive discharge experience to lead our discharge teams across our 3 sites. The post holder will be responsible for the operational management and effective, efficient delivery of complex discharge planning for all patients within the North West Anglia NHS Foundation Trust and you will be accountable for the discharge team service provided at NWAngliaFT on a day-to-day basis. They will provide flexible solution finding to support people that we care for in their discharge planning. The post holder will ensure that the patients voice is integral to all decisions about their care. The post holder will also lead their team in sharing good practice across all services and engage in active ward-based education and skill sharing.
Be accountable for the daily operational management of the discharge team.
Exercise delegated authority on behalf of the Head of Site Operations, Discharge and Patient Flow to resolve day to day management issues within complex discharge.
To ensure the effective management of patient flow.
To develop a process to identify and manage demand and capacity daily.
To manage the operational process for NWAFT complex discharge across a 7-day service.
Utilise a project management approach to lead on and implement service development, improvement, and modernisation to engage all staff and deliver sustainable change in culture and working practice.
Have a level of whole-system knowledge that facilitates communication at all levels including departmental managers, senior nursing staff, senior clinicians and managers across the Trust, external care providers, external agencies and local community providers.
