Description:

The Community Neurorehabilitation and Stroke Service have an exiting opportunity for a dynamic and skilled clinician to join our well established service, based at Retford Primary Care Centre.
As a Highly Specialist AHP (physiotherapist or occupational therapist); you will work as an autonomous practitioner, working collaboratively within an integrated multi-disciplinary team.
You will manage a complex caseload and provide expert assessment and treatment programmes for patients who require specialist neuro and stroke rehabilitation, including early supported discharge (ESD). Therapy is delivered in patients own homes for the adult population of Bassetlaw.
You will supervise and support junior colleagues, promoting and supporting career development whilst contributing to the training needs of other disciplines. You will play a key part in the development of long term condition/stroke management within community services and ensuring commissioned targets & Key Performance Indicators are met.
To assist in the development and implementation of appropriate pathways of care for neuro and stroke rehabilitation
To lead the co-ordination of care, working in partnership with patients/service user, carer, colleagues, partner agencies and key stakeholders
To deputise for the Team Lead when needed and provide support with the clinical development and management of staff

As a Highly Specialist AHP for the community neuro & stroke service; you will provide leadership and support to staff regarding the delivery of the specialist community rehabilitation programmes.
As a specialist clinician, the post holder will provide highly expert assessment and treatment programs for complex patients who require rehabilitation following a diagnosis of a long term neurological condition or stroke . The post holder will provide support to junior staff within the team to manage complex patients who require rehabilitation.
The post holder will carry a caseload of patients as an autonomous practitioner, including those with highly complex presentations, using advanced clinical reasoning.
To provide high quality holistic evidence-based care to patients/service users that is responsive, effective and efficient.
To be responsible for the clinical management of the team and service including management of waiting lists
