Guided by principles of accountability, compassion, growth mindset, partnership, and integrity, we drive excellence in all facets of our operations.  POSITION SUMMARY:  In this contractor role, the MD will be responsible for establishing the Tapestry Health patient relationship for our chronic care management services.   KEY RESPONSIBILITIES:  Responsible for establishing the Tapestry Health patient relationship for our chronic care management services.   Be available at a minimum of 10 working days per month to conduct approximately 375 visits onsite and remotely. These are brief, non-clinical judgement consults simply advising new patients about the services they'll receive from Tapestry Health.  Document patient encounters accurately and thoroughly within our platform at time of visit.   Provide end of day report to program administrators to track progress for completion of visits.  MINIMUM REQUIREMENTS:  Qualified MD/DO  Active, unrestricted, Medical Licenses in Pennsylvania and at least 4 other states. Excellent clinical judgment and decision-making skills.   Must have reliable transportation to travel to assigned buildings.   Commitment to providing patient centered care and improving health outcomes.   Familiarity with electronic health record (EHR) systems and healthcare technology.   Ability to travel within 1-2 hours of assigned region   COMPENSATION STRUCTURE: 10-20k/month, mileage reimbursement, Malpractice.
