Health Monitoring & Data Review: Reviews incoming biometric data and patient-reported information to identify changes in condition, trends, or areas requiring follow-up or escalation. Patient Outreach & Support: Engages directly with patients to provide education, encouragement, and early intervention when health readings fall outside expected ranges. Care Coordination: Communicates with primary care providers, specialists, and internal Brook team members to share insights, close care gaps, and maintain continuity of care.
