The team take a targeted approach to reduce health inequalities working with individuals and communities with the most need, situated in areas of high deprivation or those who are socially excluded. We are looking for an enthusiastic, highly motivated individual who wants to make a real difference within our communities. We will be recruiting for a Health Improvement Practitioner role to work with our Mid team on a 2-year fixed term contract supporting targeted Adult populations within the locality. We are looking for one person.
