At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families. As a 7 On / 7 Off After Hours Registered Nurse, you will: Complete an initial assessment of patient and family to determine hospice care needs Provide a complete physical assessment and history of current and previous illness Use health assessment data to determine nursing diagnosis Triage calls and assists patient/primary caregiver with problem resolution; counsel the patient and family in meeting nursing and related needs Make visits for symptom management and support as needed Modify patient treatment plans as indicated by patients' responses and conditions Maintain current, accurate, detailed reports in accordance with policy Monitor, record and report symptoms and changes in patients' conditions to the IDG Provide health care instructions to the patient as appropriate per assessment and plan Obtain and receive physician orders as required Pronouncement of patients Other duties as assigned Requirements Current license to practice as a registered professional nurse in Georgia Current and continued CPR certification At least one or two years of recent home care experience; hospice experience highly preferred Good verbal and written communication and problem-solving skills Responsible valid driver’s license; reliable, insured, automobile for making home visits required
Responsibilities
The Registered Nurse will complete initial and physical assessments to determine hospice care needs, triage calls, and provide problem resolution and counseling to patients and families. Responsibilities also include making necessary visits for symptom management, modifying treatment plans, and maintaining accurate patient reports.
