We’ve combined core functions from two key roles and created a split position divided between patient care and quality improvement opportunities. Rolled on top of this are flexible Part time 8-hour workdays or Full-time10-hour workdays supporting a 7 days on, 7 days off schedule. Collectively all these ingredients result in a position offering a motivated nurse unparalleled time away for family or adventure. Weekdays focus on being a great nurse providing care with a high-level nursing skill proficiency. Weekends will wind down the 7-day cycle with referral reviews and liaison work between Nascentia Health and providers we regularly work. Let’s design the rest of your dream role together!

REQUIREMENTS:

  1. Current license to practice as a Registered Professional Nurse in New York State or degree in a health-related field.
  2. Minimum one (1) year home health care experience preferred.
  3. Current New York State driver’s license and automobile insurance required.
  4. Excellent verbal communication skills required.

PHYSICAL REQUIREMENTS:

  1. Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.

  2. Ability to perform tasks involving physical activity including:

  • Pushing/pulling - Typical weight 50 pounds
  • Maximum weight 150 pounds
  • Lifting/ carrying - Typical weight 25 pounds
  • Maximum weight 40 pounds
  • Patient transfers, extensive bending, standing, kneeling, walking in various weather conditions e.g. snow, rain, sleet, and going up and down stairs.
  1. Ability to drive in all weather conditions.
  2. Visual/hearing ability sufficient to comprehend written/verbal communication.
  3. Requires standing or walking approximately 75% of day.
  4. Hearing/speaking ability sufficient to communicate on the telephone and with families, physicians and hospital staff.
  5. Ability and willingness to drive in all kinds of weather conditions to make facility visits.
  6. Bloodborne Pathogens Exposure Category I – Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.

Responsibilities

KEY RESPONSIBILITIES INCLUDE:

  • Weekend Duties (Saturday-Sunday): Conducting assessments and screenings to determine the appropriateness of home care services, managing referrals, and visiting facilities daily.
  • Weekday Duties (Monday-Friday): Completing patient visits in the field and ensuring the continuity of care for all assigned clients.
    This position requires a dedicated and flexible nurse who can seamlessly transition between coordinating care plans and providing direct patient care in the field.

PERFORMANCE RESPONSIBILITIES AND STANDARDS HOME CARE COORDINATOR:

  1. Promote continuity of care for referred clients.
  • a. Determine client/family home care needs upon referral for home care.
  • b. Assist in the development of a home care plan, including the arrangement of needed services, supplies and appliances.
  • c. Review home care policies and procedures with clients/families, physicians, agency care providers, insurance providers and service and equipment vendors as indicated.
  • d. Act as a liaison between Nascentia Health and hospitals, rehabs and/or SNF.
  1. Promote quality assurance as it relates to home care planning.
  • a. Document assessment and home care eligibility per Nascentia Health policies and procedures.
  • b. Act as a resource and/or consultant to hospital or SNF staff regarding home health services.
  • c. Participate in quality improvement activities relating to home care planning and coordination.
  • d. Maintain accurate and up-to-date statistics regarding home care referrals.
  • e. Participate in team conferences between client/family, physician, hospital staff and home care agency staff.
  • f. Work closely with Weekend Manager in maintaining referral expectations within the agency’s daily and future capacity projections.

PERFORMANCE RESPONSIBILITIES AND STANDARDS FIELD RN:

  1. Ensure quality and safe delivery of home health services.
  • Registered Nurse is able provide care to a diverse set of clients.
  • Follow plan of care based on client assessment and OASIS evaluation.
  • Ensure that all documentation meets quality standards:
  • Completes all OASIS evaluations.
  • Documentation reflects nursing care and interactions.
  • Documentation is clear and concise.
  • Documentation is completed in timely manner
  • Understand and follow nurse code of ethics and standards of practice.
  • Coordinate with internal and external resources to meet patient needs.
  • Maintain established productivity expectation
  • Communicate client care issues to Weekend Manager/Clinical Supervisor.
