We’re seeking a compassionate Aboriginal Health Worker to play a key role in providing culturally sensitive, trauma-informed support during health assessments for Aboriginal children and young people in Out-of-Home Care (OOHC).
In this collaborative, multidisciplinary environment, you’ll work closely with registered nurses, social worker, paediatrician and administrative support to guide carers and young people through their healthcare journey. In this role, you will be the primary point of contact for carers and caseworkers, facilitating the engagement of Aboriginal children and young people with the OOHC Health Pathway Program (HPP). You will coordinate and support health assessments, assist in the development of Health Management Plans, and liaise with external health providers, Department of Communities and Justice and other Non-Governmental Organisations (NGO’s) to ensure optimal health outcomes.
Additionally, you will attend multidisciplinary clinic appointments, particularly for cases with complex needs, and provide follow-up support to carers. Your work will be integral in bridging communication between clinical teams and Aboriginal communities, ensuring that care is delivered in a culturally respectful and effective manner.
The OOHC Health Pathway Program team includes nurse, social worker, paediatrician and administration support.
