Responsibilities

The Aboriginal Health Worker (AHW) works as part of the Out of Home Care (OOHC) multidisciplinary team to ensure culturally responsive assessment and care coordination services for Aboriginal children. The position will be responsible for supporting engagement with clients and carers, ensuring identification and consideration of cultural needs as part of assessment, supporting clinicians to provide culturally responsive care and facilitating care coordination as part of the multidisciplinary team. OOHC sits within the Staying Safe in Community Directorate and contributes to the integration of Violence, Abuse and Neglect services in the district.
The objective of the position is to reduce the disadvantages experienced by Aboriginal Children and Young People who are in OOHC due to exposure to abuse, violence or neglect. This position will assist in improving access to health services and provide a coordinated journey through the Health System via care coordination and supporting the development of a health management plan.

Staying Safe in Community Directorate staff work within a trauma informed framework with an understanding of and responsiveness to the impact of trauma on children, young people and their families. The position will ensure, services are provided in a manner that is welcoming, safe and appropriate for all, including individuals, families and communities who have experienced trauma. By adopting a trauma-informed and family sensitive approach, health services can play a key part in identifying and meeting the needs of children, young people, individuals, families and communities who are vulnerable.
