THE SERVICE & TEAM:
The Independence and Wellbeing Service was established to prevent reduce and delay demand on services. As part of this service the Access Team acts as Adult Social Care’s first point of contact (for people over 18 years) Under the leadership and management of the Team Manager to work within a team, the Senior Access Assessor will ensure that the team continues to be the first response for Adult Social Care receiving referrals, information requests and concerns from the public, other professionals and external agencies, via a variety of media.
THE ROLE:
The Senior Access Assessor role is to embed and support in the day to day management of the team to ensure there are effective processes in place to gather information and identify and initially assess people’s presenting social care needs through discussion and to decide best next steps. Best next steps could be information and advice, signposting, reablement or assessment including responding to safeguarding alerts. The information will be recorded on computerised records.
