PROGRAM DESCRIPTION:
Catholic Community Services’ Counseling, Recovery & Wellness (CReW) Program is a licensed community mental health and substance use disorder treatment program and provider of the King County Integrated Care Network (KC-ICN). Our collaborative team focuses on serving adults with mental illness and/or substance use disorders to help improve the quality of life for our clients and our great community. Services offered utilize Harm Reduction and Recovery principles, with an emphasis on building collaborative relationships with the persons served as the first step towards lasting engagement in treatment. Range of services includes diagnostic assessments, individual therapy, peer support, case management, group counseling, skills training, care coordination, medication management, and psychoeducation. Our team was created out of the need for onsite supportive services in our agency housing program and we adapt our services to best fit the unique needs of our clients in a variety of locations within Seattle downtown and greater King County. POSITION DESCRIPTION:
This position serves as the first clinical point of contact for adults seeking referral and access to outpatient behavioral health services. The Access Coordinator is responsible for determining eligibility and facilitating access to behavioral health services. This is accomplished by fielding and tracking all referrals, utilization and response timeframes as well as by collecting a range of demographic and health care related information. This role holds primary responsibility for answering the program’s main phone line, fielding inquiries about referrals, enrollments, and services offered by the program. The Access Coordinator will create clinical records and enter data into an Electronic Health record as appropriate and will initiate the biopsychosocial assessment by meeting with individuals to administer clinical assessment tools that can inform the comprehensive diagnostic picture, which is completed by a Mental Health Professional on the team. The Access Coordinator will continue to engage with individuals, providing support and referrals as needed throughout the clinical assignment process. Work is performed under the supervision of the Site Director or Clinical Manager.
MINIMUM QUALIFICATIONS:
• Bachelor’s degree in social work, psychology, behavioral sciences or equivalent field.
• 1 year of clinical experience working with adults including knowledge of behavioral health and homelessness continuum of care.
• Demonstrated clinical engagement skills which may include Motivational Interviewing and Collaborative Problem Solving.
• Computer proficiency in Excel and Word and knowledge of Behavioral Health (EHR) electronic health record systems.
• Demonstrated strength in community capacity building and collaboration.
• Ability to prioritize and effectively handle competing demands in a fast-paced environment.
• Demonstrated clear and concise professional written and verbal communication.
• Must have an acceptable driving record per CCS’ driving policy (required to provide current driving abstract).
• First/Aid CPR certified or the ability to become certified within the 1 month of employment.
• Applicant must successfully pass required background checks prior to an offer of employment.
• Ability to work both independently and as a member of a team.
