Description Sangre de Cristo Community Care (SDCCC) is seeking an experienced Part Time Account Manager to join our dynamic marketing team serving the Trinidad/Walsenburg area. As Southern Colorado’s leader in supportive, palliative, and end-of-life care, we are dedicated to enhancing quality of life while honoring the dignity and integrity of individuals and their families through expert symptom management, compassionate support, and exceptional care. Position Summary The Account Manager is responsible for driving referral growth and developing strong relationships within an assigned territory. This role serves as a key liaison between Sangre de Cristo Community Care and healthcare providers, community partners, patients, and families. Responsibilities: Responsible for the overall development and referral growth of assigned territory, including facilities, physicians, care providers, and community agencies. Develops, implements, and evaluates quarterly and annual territory plans to achieve growth goals and to implement agreed upon strategies and actions. Develops business relationships with providers by making effective sales calls and presentations to targeted referral sources. Maintains professional and clinical knowledge of provider services. Acts as liaison between referral partners and SDCCC to communicate information about new products, programs, and service delivery. Meets with patients and families to discuss home health and/or hospice and palliative care services, individual needs/concerns, and manage expectations. Takes the initiative in building relationships within the admissions and marketing department and with the appropriate patient care resources staff to assure the establishment of effective communication with referral sources and internal stakeholders. Updates and maintains customer and competitor profiles, logs daily call activity in CRM, and documents plans for follow up activities with referral sources. Serves as agency staff liaison with physicians, facility nurses, social workers, discharge planners and community agencies; collaborates with various community agencies to facilitate the delivery of appropriate services for potential patients. As requested by the Director of Business Development, participates in marketing efforts for the purpose of educating the healthcare community about hospice programs, including physician calls, lunch and learns, health fairs, speaker’s bureau, and CEU programs for healthcare providers. Prepares monthly tracking reports on referral sources and keeps agency informed of key marketplace changes relating to providers and competitors. Understands that teamwork is a crucial part of our business, and can collaborate well with colleagues in and out of the marketing department. Bachelor’s degree in marketing or related field or equivalent professional experience; and 2. Minimum of two (2) years of experience in hospice or home health sales or a related industry with a proven record of achieving incremental growth in direct sales. 3. Must show Proof of a Tuberculin Skin Test (PPD), TB blood test, and/or CXR, or get one upon employment (An annual assessment for symptoms must be completed thereafter) Proof of immunizations for MMR (X 2 if born after 1957), D/T and Varicella is recommended, but not required It is a requirement for all Sangre employees to be vaccinated against annual influenza. 4. Must possess a valid driver’s license issued by the state of Colorado. Maintain insurance in accordance with state and/or organization requirements and maintain automobile in good working order Must meet minimum driver standards; driver’s license must have no physical restrictions other than corrective lenses; no more than two moving violations in the last 36 months and no major charges, such as reckless driving, driving while intoxicated/driving under the influence of drugs, in the last five (5) years.
