Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky’s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to individuals served. Clinicians support individuals with mental health challenges in Residential, Supported Housing and Community settings. They complete all screening and enrollment of persons served in compliance with DMH regulations, including initial and ongoing assessments for risk and protective factors. In addition, the Clinician will develop and implement Comprehensive Assessments and Individualized Treatment Plans using Person-Centered Planning. Other Key Responsibilities: Collaborate with applicable Care Coordination Entities, Emergency Service Providers, outpatient clinics, and other providers/entities to share information and ensure that the needs of the person are fully met. Develop productive affiliations and partnerships with other community-based providers to enhance the agency's service delivery network. Conduct individual sessions, facilitate support groups and treatment planning meetings with individuals served. Provide clinical consultation and training to staff on the integrated team. Participate in On-Call rotation and critical incident review process with potential on-site response if needed. Qualifications Master’s Degree in Social Services field, required. Must be a Licensed Practitioner of the Healing Arts or licensed eligible with the following criteria: LCSW - must be licensed within 6 months of hire LADCI - must be licensed within 1 year of hire LMHC - must be licensed within 2 years of hire Previous professional clinical work with experience in crisis intervention and residential work highly preferred. Valid driver’s license, and acceptable driving record, required. Open Sky is proud to have a competitive compensation model for clinicians.
