An exciting opportunity has arisen to join our innovative dynamic frailty team. As an experienced and motivated Band 8A Advanced Clinical Practitioner you will be passionate about service development and providing a high standard of patient centred care.
Delivering admission avoidance of frail older people in the community, preventing hospital admission, frailty in reach and to support the development of a front door frailty same day assessment unit. Supporting frail older patients return home (discharge to assess)
The successful post holder will work as a dynamic and proactive ACP within our very successful and developing Integrated Frailty service. The service operates over 7 days and provides the frail population of Bradford the benefit of a holistic Comprehensive Geriatric Assessment.
You will be based at St Lukes Hospital.
As a team member you will be part of a friendly, supportive, and professional integrated team which comprises of Geriatricians, Nurses, Occupational Therapists, Physiotherapist, Co-ordinators, Advanced Clinical Practitioners, Pharmacists, Social services, and Community services.
The post holder will be an autonomous practitioner who will take an active role in responding, stabilising and make safe element of acute care pathways by delivery of advanced assessment, and the management and treatment of frail older patients and will also be proactive in admission avoidance, urgent care response. You will support the development of front door frailty/ frailty SDEC.

The post holder will provide clinical expertise and leadership to the integrated clinical team, drawing on clinical knowledge to undertake advanced assessment, diagnosis and treatment, inclusive of independent prescribing
The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the Care of the Elderly Virtual Ward MDT team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
The post holder will provide support and facilitation to trainee nurse practitioners and will provide a consultancy and advisory function to the MDT, utilizing expert knowledge and skills.
The role will also provide a teaching and education service to students, medical and nursing staff.
The post holder will be responsible for providing support to the development of an integrated care pathway. Liaising with social care and primary care colleagues to support admission avoidance. in liaison with other clinicians and partner organisations.
The post holder will be responsible for the operational management, performance monitoring , professional leadership, service improvement and development of the virtual war
