Job Summary: The Care Coordinator/Specialist provides psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions utilizing a recovery based and trauma-informed perspective. Care Coordinators/Specialists operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams and cultivate a clinical skill-set by forging therapeutic relationships with diverse community members. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone.
Care Coordinators/Specialists work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes. Care Coordinators/Specialists participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning. Care Coordinators/Specialists may provide all or some of the following: deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need. Care Coordinators/Specialists may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidenced-based interventions or therapeutic modalities. Although staff members in this position are not typically LPHA’s, staff members in this position are working under the supervision of a LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment.
On-call responsibilities may be required dependent upon unit protocols. This position may have additional duties associated with providing bilingual communication services and requires the completion of a language competency test.
Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

EDUCATION:

Required:

  • Graduated from an accredited college or university with a Bachelor’s degree with a major specialization in Human/Behavioral/Social Services or related field. Master’s degree preferred.
