Job Summary: Assists activities directors by planning, organizing, developing and implementing daily therapeutic activity programs to meet individual patient needs and interests. Adheres with current federal, state and local standards, guidelines and regulations that govern long-term care and assisted living facilities. Job Requirements: Education and Work Experience: High School Education/GED or equivalent: Preferred Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred Licenses/Certifications: Valid Driver’s License (DL) and must be at least 21 yrs of age or older: Required Department Specific License/Certifications: Certified Nurse Assistant (CNA): Required Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required Essential Functions: Assists in developing, maintaining, evaluating and directing activity programs. Participates in community planning related to facility interests, family and resident needs. Coordinates activities with other departments, as necessary. Assists with developing volunteer activity assignments, makes routine resident visits and assists with crafts and projects, as necessary. Assists in-room residents by visiting, writing letters, running errands and making appointments. Involves residents and family in planning facilities, as necessary. Provides residents with daily activities. Recommends equipment and supply needs for the department, as needed. Serves on, participates in and attends various facility committees, as appointed. Assists with activity care plan and assessment scheduling. Observes resident attendance, mood, behavior and degree of involvement. Assists by arranging transportation, as necessary. Performs administrative requirements, including form and report completion. Assists with admission, discharge and other documentation. Drives the company vehicle for resident outings or appointments and accompanies residents on trips outside of the community. Performs other job-related duties as assigned.
