Description

We are looking for a caring and passionate Activities Coordinator to join our team in Bailey’s Nursing Home. This will be a full time role of 30+ hours per week and include some weekends.

PROFESSIONAL KNOWLEDGE

· Knowledge of Residential Care legislation including:
· HIQA’s National Standards for Residential Care Settings for Older People in Ireland, 2016 and others as applicable.
· S.I. No. 415 of 2013 Health Act 2007 (Care and Welfare of residents in designated centres for older people) Regulations 2013.

Responsibilities

PURPOSE OF THE POST

To develop and facilitate programmes which meet the needs of the entire resident population in Bailey’s Nursing Home, and implement the programming model

ROLES AND RESPONSIBILITIES

The Activities Co-ordinator shall:
· Promote and maintain resident safety and provide quality care through respecting the dignity, privacy, spiritual beliefs, customs, values and rights of the resident.
· Liaise with the Director of Care regarding the provision of opportunities for residents to participate in activities in accordance with their interests and capacities. Particular consideration is given to residents with dementia and/or other cognitive impairments.
· Liaise with the Director of Care regarding the purchase and maintenance of activity equipment and other resource needs of the programme.
· Submit an annual budget and activity plan for the provision of services for the following year.
· Oversee, coordinate and provide opportunities for indoor and outdoor exercise and physical activity, personal development, communication and other psychosocial development for the residents of Bailey’s Nursing Home.
· Coordinate and plan events such as major holidays and parties in the Bailey’s Nursing Home.
· Undertake a complete activity assessment of new residents upon admission and to use this information to provide a programme that matches the needs and likes of the resident.
· Assess, plan, implement and evaluate the resident’s needs and outcomes of the individual/group activity programme. Provide suitable group and individual activities to residents designed to maintain and improve their quality of life.
· Review activity assessments on a regular basis or on a needs change basis.
· Encourage involvement from the residents of Bailey’s Nursing Home, however, the residents shall be given a choice to participate in individual and/or communal recreational activities.
· Maintain accurate documentation of activities, participation rates, and evaluation of plans.
· Participate in regular reviews and provide evidence of review of the activity programmes.
· Record all resident activities offered, completed, or refused by the resident within the resident records.
· Work within the Multi-disciplinary Care Team of Bailey’s Nursing Home to provide holistic care and services for the residents.
· Provide support and assistance to maintain the quality of care for older people, including participation in workgroups or committees as required.
· Support the development and implementation of the risk management activities, including involvement in the Service and Care Provision Risk Management Register and Individual Risk Management Plans as deemed required.
· While performing duties, ensure that resident’s privacy and dignity are maintained.
· Identify the needs of the collective group and individuals and tailor the activities to suit them.
· Know each resident by name and establish and maintain appropriate relationships with residents and their family members, which include maintaining effective communication with residents, family members, volunteers and other members of the resident’s Multi-disciplinary Care Team.
· Ensure up-to-date information on activities is circulated to each resident, or his/her representative, in formats suited to his/her capacities.
· Report any concerns about a resident’s changes in health and/or activity status to the Clinical Nurse Manager/Director of Care.
· Attend training courses both in-house and at other locations to meet and enhance the service being provided by Bailey’s Nursing Home. Participate in mandatory training as required by Bailey’s Nursing Home.
· Undertake such other duties as may be assigned from time to time by Clinical Nurse Manager.
· Ensure that all enquiries regarding care are directed to the Clinical Nurse Manager/Director of Care.
· Respect the dignity and privacy of the residents. Be aware of the confidential nature of the work and in particular, the right of residents to confidentiality. To this end the workings of the Bailey’s Nursing Home or its residents shall not be discussed with people outside Bailey’s Nursing Home.
Be responsible for the following in relation to Education and Training:
· Develop and maintain competency levels necessary for professional practice.
· Attend training courses both in-house and at other locations to meet and enhance the service being provided.
· Attend mandatory training and keep up-to-date with developments relevant to the role, be clinically responsible for practice and seek support and feedback on operational developments.
Be responsible for the following in relation to Quality and Safety Management System:
· Support and implement the Quality and Safety Management System, including resident involvement. The Quality and Safety Management System shall encompass ongoing monitoring of the service and care provided within the Bailey’s Nursing Home, including internal auditing of processes.
· Support the development and/or revision of policies and procedures in line with applicable laws, standards and best practice.
· Initiate and participate in evaluations, audits and quality initiatives relevant to the Bailey’s Nursing Home.
· Use audit outcomes to improve service provision within the Bailey’s Nursing Home.
