GENERAL SUMMARY:

Under the supervision of the Program Director, the Activity Coordinator of Iredell Adult Day Services (IADS) assesses the needs, interests, and capabilities of participants and develops recreation plans. In addition, collaborate with other Activity Assistants to plan, organize, direct, and participate in a comprehensive recreation program tailored to the general and individualized needs and limitations of frail elderly program participants.

QUALIFICATIONS:

1. Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

  1. Education:
  • High school diploma or equivalent is required.
  • Cardiopulmonary Resuscitation (CPR) certification is preferred.
  • CNA certification is preferred.
  1. Experience:
  • A minimum of one year of experience working with the frail or elderly population is required.
  • A minimum of one year of experience with Elderly Activities.
  1. Competency:
  • Position specific competencies for the Activity Coordinator will be met prior to assuming participant care.

Demonstrated creative abilities to plan, develop, and implement varied activities to meet
diversified needs.
Ability and experience working with volunteers.
Good working knowledge of dementia disease process.
Familiarity with illnesses common to frail older adult population.
Proficient in Microsoft Word.

Responsibilities

ESSENTIAL FUNCTIONS:

  1. Plans and implements group and individual activities to improve or maintain the participants’
    cognitive, physical, social, psychosocial, and artistic needs.
  2. Plans and implements activities for dementia-specific population. Oversees daily flow of dementia activities area. Activities may focus on small groups, one-on-one programming, and sensory stimulation.
  3. Organizes community outings and special events at IADS. Duties include arranging for participant transportation, entertainment, catering and involving other staff.
  4. Develops a monthly calendar of activities. Maintains record of monthly calendars as required for review.
  5. Evaluates program plans to promote effective programs according to client abilities and preferences within the IADS daily flow of events.
  6. Provides recreational educational and training to support Activities Coordinator, and other IADS staff members, as appropriate.
  7. Provides ongoing support and training to all staff involved in dementia specific programming related to the dementia disease process.
  8. Maintains participants’ medical record and fulfills charting and reporting requirements.
  9. Provides leisure education to participants and caregivers for activity modification, use of adaptive equipment, activity skill development, and recommended supplies for in-home use.
  10. Maintains general order in the activities area, including daily clean-up, routine supply inventory ordering, petty cash, and overall general budgeting.
  11. Assists participants with non-skilled Activities of Daily Living (ADL) as needed.
  12. Protects privacy and maintains confidentiality of all company procedures, results, and information about employees, participants, and families.
  13. Participates in and supports Quality improvement Initiatives.
  14. Maintains safe working environment. Follows all IADS Policies & Procedures, and Occupational Safety and Health Administration (OSHA) safety guidelines.
  15. Performs related duties as required.

GENERAL RESPONSIBILITIES:

  1. Supports the Mission of the facility and adheres to the Corporate Compliance Code of Conduct.
  2. Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity, and well-being of all participants in a safe, secure environment.
  3. Supports, cooperates with, and implements specific procedures and programs for:
    a. Safety, including standard precautions and safe work practices, established fire/safety/disaster
    plans, risk management and security, report and/or correct unsafe working conditions, equipment
    repair and maintenance needs.
    b. Confidentiality of all data, including participant, employee and operations data and reports
    breaches immediately to the HIPAA Privacy or Security Officer.
    c. Quality Assurance and compliance with all regulatory requirements.
    d. Compliance with current law and policy to provide a work environment free from harassment and
    all illegal and discriminatory behavior.
  4. Supports and participates in common teamwork:
    a. Cooperates and works together with all co-workers; plans and completes job duties with minimal
    supervisory direction, including appropriate judgment.
    b. Uses tactful, appropriate communications in various situations.
    c. Follows up as appropriate with supervisor, co-workers, or participants regarding reported
    complaints, problems, or concerns.
    d. Promotes positive public relations with participants, caregivers, and guests.
    e. Adheres to attendance policies, including checking in and out for mealtime, following break
    schedule, and seeking supervisor’s approval prior to working overtime.
    f. Completes requirements for in-service training, uniform and dress codes including personal
    hygiene, and other work duties as assigned.
  5. Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects.
  6. Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances.
  7. Assists participants and caregivers in filing grievances.
