Description Brandywine Counseling and Community Services is seeking an Administrative Care Manager for our PCCP Program! Who we are: Since 1985 Brandywine Counseling & Community Services, Inc. We promote hope and empowerment to persons and families touched by mental illness, substance abuse, HIV and multiple occurring diagnoses, and their related challenges. What You’ll Do as an Administrative Care Manager with PCCP: Play a vital role in balancing care management, staff supervision, and administrative responsibilities while ensuring effective and compassionate service delivery across all program areas Ensure that outreach staff, counselors, and volunteers provide quality, sufficient, and appropriate support services to all BCCS clients engaged in the project. Oversee the daily operations of the PCCP, including mobile outreach and the coordination of weekly outreach sites Provide onsite supervision of staff and volunteers and is responsible for recruitment, training, and scheduling Coordinate and deliver individual and group education and prevention activities, assists with the development and implementation of presentations and trainings, and provides case management services that include client assessments, service planning, referrals, and follow-up to ensure continuity of care Administrative duties are a key part of the position and include documentation review, data entry, compliance reporting, and record management Monitor supply inventory, oversee vehicle operations for outreach, and ensure quality assurance and program evaluation activities are completed accurately and on time Work in close collaboration with Gibney Mobile Healthcare, external consultants who partner with BCCS to provide onsite medical care and clinical support during outreach activities. Ensure that this partnership functions smoothly and that clients are seamlessly connected to medical services as part of their overall care plan. Offer culturally sensitive, client-centered, and accessible services to address the health, treatment, and social service needs of individuals experiencing homelessness, those at increased risk for HIV, people who use substances, sex workers, and their partners across the state of Delaware Requirements Qualifications: REQUIRED: One year of experience working with consumers at risk for or living with HIV/AIDS and HCV, including individuals who are unhoused and/or living with substance use disorder (SUD) and mental health (MH) challenges. REQUIRED: High School Diploma or GED REQUIRED: Valid driver's license with 2 points or less Preferred: Associate degree Preferred: HIV Counseling and Testing Certification Salary Starting at $21.15/hour Commensurate based on education, experience, and certification! Schedule: Monday - Friday 8:30 a.m. - 4:30 p.m. Occasional nights and weekends: some later evenings on Thursday when PCCP goes to Sunday Breakfast Mission The compensation package for this position includes: Group medical, dental and vision coverage with low employee costs 34 paid days off annually Tuition reimbursement A retirement plan with a company match of up to 4%!
