ROLE OVERVIEW and PURPOSE: Neighborhood Healthcare’s Quality Team is seeking a volunteer to support our Healthy STAR Program, which focuses on outreach for patients with uncontrolled diabetes and elevated blood pressure. This opportunity is ideal for someone interested in healthcare, public health, or patient care coordination. Please note, this opportunity is unpaid. RESPONSIBILITIES: * Assist the Healthy STAR team by creating culturally-tailored handouts to help patients with chronic diseases improve their nutrition. * Attend community and food distribution events with the Healthy STAR team to help educate patients and distribute handouts. QUALIFICATIONS: * Excellent interpersonal and communication skills * Ability to maintain confidentiality and handle sensitive information appropriately COMMITMENT: * Minimum of 2 days per week, with availability between Monday through Friday * Volunteers must commit to serving for at least 2 months BENEFITS: * Gain hands-on experience in healthcare administration, patient outreach, and quality improvement programs * Learn directly from experienced care coordinators and quality improvement staff * Contribute to meaningful work that directly improves patient health outcomes * Strengthen your resume and professional network within a respected healthcare organization
