JOB DESCRIPTION

Human Touch Home Care is seeking a Administrator, Clinical Manager (DON) to function as an exceptional manager responsible for the oversite and direction of care management, quality assurance, and compliance for the home care services.

QUALIFICATIONS:

  • Registered Nurse (RN) license in Colorado (required)
  • A minimum of 3 years of supervisor/manager experience in Home Health (required)
  • Minimum of 3 years of experience with Medicare and Oasis
  • Bachelor’s degree in Nursing or health related field (required)
  • Prior experience with computers and medical software, eMAR
  • Understanding of and demonstrated success in achieving and maintaining Medicare/Medicaid standards
  • Excellent organization skills with the ability to multi-task effectively
    Job Type: Full-time
    Salary: Based on experience
    Schedule: Mon-Fri 8:30am - 5:00pm

EXPERIENCE:

  • Home Health Care Management: 3 years (required)
  • RN: 2 years (required)

  • Lead, direct and support a team of Nurses and Medical Professionals with varying credentials in a fast-paced home health agency.
  • Work as a team to uphold the company’s mission, vision, and core values
  • Develop, maintain, and implement nursing policies and procedures
  • Ensure compliance with local, state, and federal regulations; as well as company policies & procedures
  • Promote and champion employees and patient safety/ well being
  • Monitor/audit staff practices and process improvement implementation for quality assurance
  • Evaluate the company’s nurse program and individual performance to ensure the best possible patient clinical outcomes and satisfaction
