Main duties include:

  • Supporting patients to understand, prepare for, and actively participate in their care.
  • Coordinating care and working closely with healthcare professionals, patients, families, carers, and external services.
  • Providing advice, information, and signposting to appropriate health, social care, and community services.
  • Supporting the delivery of preventative health care and helping patients improve their health and wellbeing.
  • Identifying barriers to care and supporting patients in accessing appropriate services and resources.
  • Maintaining accurate records and managing information in line with confidentiality and data protection requirements.
  • Liaising with multiple stakeholders to ensure agreed actions and care plans are followed up.
  • Supporting patients to improve their understanding and confidence in managing their health and long-term conditions.
  • Contributing to service improvement, quality improvement activity, and the development of effective ways of working.
  • Working collaboratively with clinical and non-clinical colleagues to support seamless patient care and positive health outcomes.
