Responsibilities

MAIN DUTIES OF THE JOB

Monitor and review STRATA referrals to identify and process work to community health and social care teams:

  • Managing a varied workload in a busy working environment. There is a requirement to be organised, prioritise workload and manage enquiries.
  • Managing risk and priority of routine referrals, responsible for making the decision of the referral pathway.
  • Review key blockages to patient flow, ensuring that patients achieve their most independent outcomes highlighting key issues to Clinical Prioritisers and to team managers.
  • Monitor and assist in maintaining mailbox, phones and Electronic Referral system.
  • Using clinical and professional systems to gather information as required to process referrals.
  • Timely and accurate escalation of non-routine pathway referrals.
  • Accurate and timely documentation of decisions on STRATA, RiO and Mosaic in line with organisational policies.
  • To maintain compliance with organisational and statutory responsibility for confidentiality.
  • Ensure proper use of the organisational resources in the course of business, ensuring minimal waste and minimal cost.
