Employment Type: Full time Shift: Day Shift Description: Eddy VNRA has a unique opportunity to work full time as part of our Liaison Team! If you are looking for a fast paced position this is for you! Primary responsibilities include: Communicating with the hospital case managers, Rehab and hospital services, community physicians, and various resource agencies Coordinating services for patients discharging home from an acute care setting Contacting physician offices, obtaining various services/orders (ie: DME, infusion) as needed for the homecare clinicians and ensuring referral information/orders are complete Documenting patient information into our EMR Working with various departments to coordinate a smooth transition to home Requirements: NYS Registered Nurse license Associates in Nursing, BSN preferred 1 year of RN experience Key skills required: Strong interpersonal skills Excellent computer skills Ability to multitask multiple patient referrals Organization Team player Strong time management and clinical skills Eddy VNRA provides a comprehensive orientation and supportive work environment. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
