Description VNA Hospice NWI is seeking: Admission RN Full-time with minimum on-call rotation New Starting Pay Scale Coverage area includes Porter, La Porte, Starke, Jasper, Newton and Lake. Mileage paid at current IRS rate, if a fleet car is not available. SUMMARY: Performs consultation and admission visit to assess patient's needs and completed admission assessment and agreement documentation for VNA Hospice NWI. Possesses understanding of Hospice program and benefits. Obtains orders for and procures immediate medications and needed DME/supplies. Performs physical assessment of patients' condition, safety, knowledge, pain, and symptoms. Initiates Plan of Care Instructs on disease understanding, progression, and immediate interventions. Documents assessment, current medications and initiates appropriate care plans. Requirements QUALIFIED CANDIDATE WILL POSSESS: Unrestricted RN license in the State of Indiana Diploma must be from an approved school of professional nursing. Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or hospice care preferred. Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred. Must be sensitive to issues of loss and grief. Excellent communication and organizational skills. Must maintain current CPR certification. Must hold and maintain a valid driver’s license, and automobile insurance. TB status must be current.
