This position serves as a key liaison among Community Liaisons, referral sources, patients, families, and our clinical team. This will be a hybrid role How you will make an impact: Admissions & Referral Coordination * Receive and process referrals from Community Liaisons, physicians, patients, families, and community partners. * Coordinate requests for patient records. * Coordinate and schedule patient and family appointments for informed consent and RN assessment visits. * Verify and upload patient records to EMR. * Ensure timely notification of new referrals, admissions, and non-admissions to the assigned interdisciplinary team (IDT) and ancillary providers. * Coordinate supply and DME provision to meet patient needs. * Follow up on patients who have not been admitted and/or have been discharged. Documentation * Maintain and organize patient charts, ensuring all documentation is complete, signed, and dated. * Transfer completed admission charts to the Team Coordinator in a timely manner. Patient & Family Support * Explain hospice services, benefits, and eligibility to patients, family members, and referral partners, as needed. * Serve as a compassionate and knowledgeable point of contact during the admission process. Team Communication & Collaboration * Inform appropriate team members of scheduled admission appointments and updates. * Communicate identified needs, issues, and potential solutions to the supervisor. * Attend and participate in staff meetings, educational programs, and community events, as requested. The Experience and Skills You’ll Bring * Active, unrestricted Registered Nurse License * Be available at all times during agency operating hours or as needed * The ability to make sound professional clinical judgment * Excellent verbal and written communication skills * Proficiency with EMR and data entry systems * Hospice experience preferred
