ADMISSIONS MANAGER – FULL TIME/EXEMPT

Falcons Landing is looking for an Admissions Manager for our Johnson Center Nursing and Assisted Living facility.
Falcons Landing is a Life Plan Retirement Community located west of Washington, DC in Loudoun County, Northern Virginia. We are a first-class Life Plan Community, privately held not-for-profit organization which provides an outstanding and caring work environment. Falcons Landing has a wonderful combination of residential homes, spacious apartments and three high class healthcare residences.

QUALIFICATIONS:

  • Minimum of three (3) years continuous employment as an Admissions Coordinator in a Nursing Home or Assisted Living
  • Proficient knowledge of basic computer skills including Word/Excel/Internet/Outlook is necessary
  • Experience with insurance verification and authorization processes preferred Living community
  • Associates Degree or BA desired

Responsibilities
  • Responsible for receiving referrals and inquiries from hospitals, families, and other referral sources
  • Assist with marketing activities
  • Coordinate tours of both nursing home and Assisted Living
  • Lead the admissions process in accordance with company procedures
  • Conduct pre-admission evaluations independently and in conjunction with designated facility representation
  • Insure appropriate placement of a patient or resident
  • Conduct person-to person calls outside the facility to insurance, financial, legal, and medical professionals, senior organizations, and hospital discharge planners.
  • Verifies all insurances/payer sources for potential Admission/referral
  • Alert appropriate department managers and facility team of projected admissions, room changes and discharges
  • Report daily census to all appropriate staff
  • Coordinate admissions paperwork to secure completion of admission and financial responsibility forms.
  • Arrange physical, social, emotional, and support service requirements, including transportation
  • Monitor inquiry, admissions, and census trends.
  • Maintain a working knowledge of federal and state regulations and reimbursement issue.
