Overview The Admissions Nurse in a home setting, provides and arranges for hospice care for patients in a collaborative relationship with members of the Interdisciplinary team. The Admissions Nurse makes initial nursing assessments and treatment decisions based on the needs and wishes of the patient and their family. In addition, The Admissions Nurse gathers important contact information, reviews and documents the patient’s medication list and takes their full medical history. Location: Falls Church, VA Hours: PRN COVID-19 vaccine required to be completed upon start. Responsibilities The Admissions Nurse, in agreement with public health nursing standards, practices, and policies, will provide and arrange for Hospice care for patients/families. The Admissions Nurse is responsible for supervising LPNs, CNAs, Unit Secretaries, and Volunteers. May be asked to rotate to other regions/departments. Qualifications Experience RequirementsMust have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly preferred. Education RequirementsMust have a degree in Nursing from an accredited nursing program. A BSN degree is preferred. Required Certificates and/or LicensesMust have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from a compact state is also accepted. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus. Driver's License Must have a driver's license in good standing and access to a vehicle during working hours.
