Position Overview: The Admission Nurse LPN role supports the intake and transition of new residents by gathering required resident and family information and facilitating timely admissions. This position works in accordance with established policies and procedures and under the direction of the supervising nurse to promote accurate documentation, continuity of care, and a smooth admission process. * Completes comprehensive initial admission assessments, including head-to-toe evaluations, skin assessments, and required risk screenings upon resident arrival. * Reviews hospital discharge summaries, physician orders, diagnoses, and treatment plans to ensure accurate admission processing. * Performs medication reconciliation at admission, identifying discrepancies and communicating findings to the RN or healthcare provider. * Initiates admission documentation in the electronic health record (EHR), ensuring accuracy and compliance with facility, state, and federal requirements. * Administers initial or first-dose medications as ordered and monitors residents for immediate responses or adverse effects. * Identifies changes in condition or clinical concerns during the admission process and promptly reports them to the RN or healthcare provider. * Provides admission-related education to residents and families regarding facility routines, care expectations, and next steps. * Collaborates with Registered Nurses, Nurse Managers, Admissions, Therapy, and interdisciplinary team members to support a safe and smooth transition of care. Qualifications: * Must be a Licensed Practical Nurse (LPN) in Tennessee or compact. * Active BLS/CPR * Minimum of one (1) year of nursing experience in a skilled nursing facility, long-term care, rehabilitation, or comparable healthcare setting preferred. * Knowledge of admission processes, clinical documentation, and electronic health record (EHR) systems in an SNF environment.
