Avalon Care Center Bellingham is seeking an Admissions Nurse to join our team!    $43.50- $48.05/hr Monday - Friday 11:30 am - 8:00 pm   We are looking for individuals who embrace a reverence for life and a heart for healing. We offer great challenges, a rewarding career and opportunities for advancement!   Full-time are eligible for: * 401K * Medical, Dental & Vision * FSA & Dependent Care FSA * Life Insurance * AD&D, Long Term Disability, Short Term Disability * Critical Illness, Accident, Hospital Indemnity * Legal Benefits, Identity Theft Protection * Pet Insurance and Auto/Home Insurance. **Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.**   Responsibilities Admissions Process * Complete admission assessments for incoming residents, including physical and clinical evaluations. * Coordinate admissions to ensure a smooth transition into the facility. * Review hospital records, physician orders, medications, and treatment plans prior to admission. * Collaborate with physicians and interdisciplinary team members to support continuity of care. * Ensure accurate and timely completion of admission documentation, authorizations, and medical records. * Educate residents and families regarding admission procedures, care plans, and facility policies. * Assess residents for appropriate level of care and identify immediate clinical or safety concerns. * Coordinate room assignments, equipment, and resident care needs upon admission. * Maintain compliance with state, federal, facility, and HIPAA regulations. * Assist with resident transfers and discharge coordination as needed.
