The Start of Care/Resumption of Care Registered Nurse (SOC/ROC RN) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and supervising HHA's and CNAs. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care. Our Case Management model ensures you will receive timely training and support to help you provide the best care possible. Pay range: $32.00 - $45.00 hourly + Millage reimbursement at Federal rate Job Duties: Conduct initial home health care OASIS assessments for comprehensive care, including Starts of Care, Resumptions of Care, and Recertifications. Work primarily within three (3) zones, with likely additional travel time based on zone coverage Develop a plan of care based on physician orders and initial client assessment and continue to monitor, revise, and update based on ongoing assessments or changes of condition Facilitate referrals to interdisciplinary team (Physical Therapy, Occupational Therapy, Aroma Therapy, Speech Therapy and Falls Prevention) Perform medication set-ups as needed or required under the direction of the Clinical Manager and CareAparent scheduling Oversee and supervise Licensed Practical Nurse (LPN) visits and ensure plan of care is being followed and report back to Case Manager Coordinate, oversee, and supervise on-site CNA / PCA / HHA visits and ensure plan of care is being followed Report changes in client condition to physicians, Clinical Manager, and Case Manager Prepare appropriate medical documentation of client assessments and visits using CareAparent EMR platforms Perform other nursing functions as may fall under state and federal nursing guidelines, CMS, and the Joint Commission Able to work on call for rotating evenings and weekends Perform other skilled nursing visits as directed by Clinical Manager Maintain RN license annually Qualifications: Graduation from an accredited and approved nursing program; BSN preferred Minimum of one (1) year of relevant home care or assisted living experience Valid Registered Nurse license in the state of Minnesota. Valid driver’s license with an insured, reliable vehicle. Knowledge regarding quality improvement and home health standards of care. Knowledge regarding the impact of acute and chronic illness on the lives of patients and families in the community.
