POSITION SUMMARY

Coordinates intake of referrals, obtains patient information and physician orders, performs admissions of appropriate patients, establishes initial plans of care, assists with assignments of patients to field nurse for ongoing case management, assists with team conferences, insures that patient care records are accurate and complete, shares clinical expertise with nursing staff, coordinates services with social worker, and contributes to the performance improvement program of Hospice of Lubbock. Requires minimal supervision.
Charge/Lead:
Assists in the coordination of clinical services for Hospice of Lubbock. Ensures coordination with referrals, obtains patient information and physician orders, performs admissions of appropriate patients, establishes initial plans of care, assists with assignments of patients to field nurse for ongoing case management, assists with team conferences, insures that patient care records are accurate and complete, insures coding of patient diagnosis, shares clinical expertise with nursing staff and other team disciplines, and contributes to the performance improvement program of Hospice of Lubbock. Serves as the Alternate Nursing Supervisor when Clinical Manager is unavailable. Requires minimal supervision.

EDUCATION AND/OR EXPERIENCE

  • Required - Coursework/Training 18 hours of Hospice training.
  • Required - Coursework/Training 6 hours of Bereavement training.
  • Preferred - Bachelor’s Degree Nursing.
  • Preferred - 2 years Nursing experience.
  • Required - Experience in working with terminally ill patients.

Responsibilities


    • Receives and processes intake information on patients referred for hospice care to include determination of appropriateness of admission based on Hospice of Lubbock admissions criteria.



      • Initiates coordination of services for new admissions such as securing durable medical equipment, supplies and prescription items.



        • Assists with team conference as requested.



          • Conducts team conference with or in the absence of the Clinical Supervisor.



            • Contributes to performance improvement and standards by monitoring documentation in patient charts to assure completion and accuracy.



              • Make recommendations on policy and/or procedure additions and changes as necessary.



                • Shares clinical expertise with patient care staff.



                  • Obtains physician signatures and those of other disciplines according to policy.



                    • Submits documentation in a timely manner.



                      • Participates in the on-call schedule.



                        • Charge/Lead:



                          • Coordinates clinical activities and support measures for hospice patients by ensuring appropriate admissions, resources appropriate staff with tasks and assisting with logistical arrangements in support of patient care.



                            • Secures for new admissions plan of care requirements such as durable medical equipment, supplies and prescription items.



                              • Insures the coding of patient diagnosis and develop the initial scope of the plan of care to meet all regulations and guidelines for quality hospice care.



                                • Oversees the Intake LVN in making assignments to interdisciplinary team members.



                                  • Utilizes available staff in most efficient manner in collaboration with Clinical Manager, aware of agency’s productivity requirements and staffs accordingly.



                                    • Support Clinical Manager with phone, coverage, takes MD orders and writes a verbal order, reports all new orders to Clinical Manager and pertinent IDG team members.



                                      • Assists in staffing all visits in accordance with visit frequency and authorization.



                                        • Reports to Clinical Manager or Director any visit discrepancies, order discrepancies or plan of care needs.
