JOB DESCRIPTION

Position Title: Assistant Director of Nursing (ADON) / MDS Coordinator
Reports To: Administrator and Director of Nursing
Facility Type: Long-Term Care / Skilled Nursing Facility

POSITION SUMMARY

The ADON/MDS Coordinator is a dual-role leadership position responsible for assisting the Director of Nursing in managing clinical operations and overseeing the facility’s Resident Assessment Instrument (RAI) process, including completion of the Minimum Data Set (MDS). This individual ensures compliance with federal and state regulations, supports quality of care initiatives, and helps maintain efficient clinical operations while promoting resident-centered care.

QUALIFICATIONS

  • Current RN license in good standing (required).
  • Minimum of 2 years of nursing experience in a long-term care setting (required).
  • Previous experience with MDS 3.0, care planning, and RAI process (required).
  • Leadership or supervisory experience (preferred).
  • Strong knowledge of federal and state LTC regulations.
  • Excellent communication, organizational, and leadership skills.
  • Ability to manage multiple responsibilities and prioritize effectively.

PHYSICAL REQUIREMENTS

  • Ability to sit, stand, bend, and walk intermittently throughout the day.
  • Must be able to lift up to 25 lbs. occasionally.
  • Visual and auditory acuity for assessing residents and reviewing documentation.
