Description AMHC is seeking a compassionate, organized, and motivated Full Time Adult Behavioral Health Home Coordinator / Community Integration Specialist to support adults living with long-term psychiatric disabilities in achieving stability, independence, and wellness within their communities. Key Responsibilities Build strong therapeutic relationships, including engaging hard-to-reach individuals Assist clients in identifying personal wellness goals and developing skills to achieve them Coordinate and manage services related to each client’s Plan of Care (Section 92) or Individualized Support Plan (Section 17) Provide supportive counseling, decision-making assistance, and crisis intervention as needed Develop and facilitate skill-building and psychoeducational groups Coordinate care with physical and behavioral health providers Assist clients in accessing community resources, including accompanying clients when appropriate Support families and significant others (with client authorization) Maintain accurate clinical and administrative documentation Participate in team meetings, supervision, and staff development activities What We’re Looking For Strong interpersonal and communication skills Ability to work effectively with individuals from diverse backgrounds Comfort working both independently and as part of a team Flexibility to meet client needs, including occasional extended hours Commitment to confidentiality, ethics, and professional standards Work Environment & Requirements Work occurs in office, community, and client home settings Regular travel within the service area is required Ability to sit, stand, walk, and lift up to 25 lbs Must be able to use standard office equipment and computer systems Why Join Us?
