Are you a dynamic, enthusiastic, flexible, creative individual with a Degree or Diploma in a Health-related field such as Activation, Therapeutic Recreation, Social Service Worker, Gerontology, or Occupational/Physical Therapy Assistant?
Do you have experience working in a Community-based setting?

Providence Healthcare offers a unique, community-based, Adult Day Program, 24 hours a day, 7 days a week. Our home-like and social program operates 365 days a year, including statutory holidays and offers flexible hours, caregiver relief and meaningful activities for individuals with moderate to severe dementia.
As a member of the Adult Day Program interdisciplinary team, the Clinical Resource Coordinator is responsible for providing direct service-delivery for our clients who attend the Adult Day Program, through service coordination, personal care and recreational programming, in accordance with Providence Healthcare’s Mission and Values.
