QUALIFICATIONS

  • Post-secondary degree in social work or a related field of study or equivalent combination of education and experience over a minimum five-year period (foreign credentials and relevant experience will be considered)
  • Minimum of five years of experience in the social service/developmental service field
  • Minimum of three years of case management experience working with individuals with intellectual/developmental disabilities and families
  • Demonstrated clinical assessment, formulation, planning, intervention, and evaluation skills
  • Experience facilitating group education sessions
  • Demonstrated knowledge of developmental services and community resources in Toronto
  • Demonstrated commitment to person-directed service
  • Strong advocacy skills
  • Demonstrated ability to engage in creative problem solving and decision making
  • Demonstrated ability to foster independence in others
  • Strong interpersonal and communication (verbal, written and listening) skills
  • Strong organizational, time management and computer skills, including familiarity with database applications.
  • Demonstrated ability to effectively collaborate within the developmental service sector, as well as across sectors, for the supports clients need and to build capacity of others.
  • Demonstrated commitment to principles of social justice, anti-oppression, equity, and inclusion
  • French as a second language considered as an asset
  • Dual Diagnosis experience considered an asset
  • Ability to travel regularly to client home visits, community meetings, agency locations etc. within the City of Toronto
  • Ability to work flexible hours as required
  • Valid First Aid and CPR and CPI considered an asset
  • Satisfactory Criminal Reference Check with Vulnerable Sector Check

Responsibilities

RESPONSIBILITIES

  • Short term case management focusing on developing plans with eligible clients to secure housing
  • Assesses client needs from a bio-psycho-social framework
  • Works with the client in the process of developing a comprehensive, goal-oriented, time-limited plan, which builds on strengths and addresses client identified needs
  • Monitors the implementation of the client’s plan and reviews its effectiveness with the client and their family on a regular basis
  • Supports the client to locate, access and evaluate appropriate supports and community resources (e.g., personal support workers, day programs, residential options)
  • Collaborates with a wide variety of service providers and community resources
  • Advocates with the client and their family for appropriate resources
  • Ensures continuity of service
  • Facilitates resources and referrals as determined by needs of clients
  • Attends and participates in team meetings, sector meetings, community planning/partnership tables and supervision meetings.
  • Identifies and reports high risk situations and serious occurrences as required
  • Ensures the regular and timely recording of case notes, client and service data as required by funders and Family Service Toronto’s policies
  • Attends case consultation meetings and training and staff development opportunities
  • Implements and participates in program evaluations and client record audits
  • Delivers service and conducts work in accordance with Quality Assurance Measures and accreditation requirements
  • Supervises students and/or volunteers
  • Other assigned duties as required
