JOB DESCRIPTION

This position supports young adults (18+) transitioning from congregate care to independent living at our residences located in the Durham Region. The Adult Support Worker supports vulnerable and/or at risk adults through providing guidance with day-to-day activities increasing life skills, social networks and safety using harm reduction, therapeutic and behavioral modification models.
In addition, this position plays a significant role in supporting young adults during their community exposure.
The Adult Support Worker reports to their assigned manager.

Responsibilities
  • Ensures a safe, comfortable and welcoming environment.
  • Offer emotional support and encouragement to individuals.
  • Assist with daily living activities and needs.
  • Promotes behavioural change and personal growth of adults who are having difficulties coping as a reult of social, emotional, or cognitive delays
  • Provides life skills training as per goals set out by the adults and other stakeholders
  • Reaches out to adults at times when they are having emotional challenges, assessing their primary needs and taking appropriate action using a non-violent crisis intervention service delivery.
  • Works within the adult’s community to utilize local resources and programs
  • Initiates activities and participation in activities, related to the specific needs and goals of the adults in our program
  • Maintains accurate and current files records as required by the company
  • Maintains awareness of community resources and gaps as well as the socio-economic and geographical make-up of the community
  • Culture & diversity competent practices
  • Adheres to all Milestone Community Care policies and procedures
  • Represents Milestone Community Care in a positivie, friendly and professional at all times
  • Supports the Mission and represents the Culture of Milestone Community Care
    Preferred Qualifications
    Diploma in a related human/social service field or equivalent combination of education, training and experience an asset.
    Understanding of the challenges facing the adults and their unique and varying needs
    Experience creating participant centered /support plans
    Understanding of, and experience working with adults transitioning to independent living
    Ability to work a variety of shifts to accommodate appointments, and the need to provide support 365 days per year, 24 hours per week
    Demonstrated ability to communicate effectively and adapt to accommodate developmental needs
    Ability to work effectively as part of a team.
    Compassionate and patient demeanor.
    Excellent organizational and communication skills
    Valid drivers license and access to a vehicle
    Criminal Reference Check with Vulnerable Sector Screen
    Valid emergency first aid certificate with CPR
