Targeted case management will be provided for other groups who are identified as being at elevated risk due to social factors, including but not limited to, those indicated by the Social Determinants of Health, racial inequities, and/or age. The adult targeted case manager performs comprehensive assessments to identify patient needs and assist them in gaining access to indicated medical, social, legal, educational, housing, vocational, and other services and supports with the goal of empowering consumers to regain control over their own lives while promoting the recovery process.
